Another day, another drive around Google Street View, checking the route for a #NHS appointment. This time for my NHS #Covid #vaccine (#13). I may be the only under 75 patient there again! Sooner than originally booked for me. Important, given I'm due major surgery, one way or another.

It is amusing to count roundabouts for directions. We were last there in November. Not the main #Dundee vaccination centre, quite a bit closer to our home. But still a bit of an adventure to get to successfully.

The crisis in care for the elderly in one graph.

In the last ten years while the population of the over 75s has expanded (reflecting the slow if sometime halting rise in life expectancy & better medical outcomes for the elderly), so care beds per thousand of the population of over 75s has fallen.

If you're wondering why hospitals have difficulty discharging the elderly into care, this may not be the only factor, but its also hardly insignificant.

Anyone who thinks the two stories related in the piece below about disabled people being marooned in hospital as their integrated home care package cannot be agreed, has nothing to do with budgeting is missing what is going on in the engineered crisis of the NHS & the care sector.

As always its vulnerable people who are trying to live their best lives (but needing support to do so) who are the target of spreadsheet wielding managers.

Roy Lilley (NHSMangers.net) sums up the plight of the NHS well....

The fact that this implies the usual critique of the politicisation of the NHS doesn't make such a summary any less salient.

Only when the NHS stops being subject to imposed continual reforms & reorganisation, only when it is funded & staffed properly & not subject to the whims of politicians with 'the answer' can we hope that our health service can once again be the service we acutely need.

I've said before that the NHS crisis is largely made up of a workforce planning problem - not enough staff attending patients.... but, now it seems that the NHS, under continuing budgetary pressures, plans to reduce recruitment & mobilise AI & associated technologies in treatment.

This is of course Wes Streeting's doing, so one can but hope that it is now recognised for the miss-step it is.

(The new plan is also focussed on staff retention which *is* welcome)

I got a phone call from the GP today about my ongoing blood pressure issues.
He doesn't want me to have medication, and is content for me to monitor it, despite the average readings being into hypertension territory.
He is arranging for an ECG and at the same time wants me to be aware of the outside possibility of atrial fibrillation after exercise as a cause of my fatigue.
I'm taking this as good news.

We know weak workforce planning & staff shortages are a major element in the continuing NHS crisis, so its unsurprising nurses see this problem as part of their every day experience.... and the thing about health *care* is in the end its only really going to be as good as the people working & their ability to focus on the care of patients - it really is a numbers game.

Perhaps now West Streeting is gone, the idea that technology is the answer will fade?

While about three fifths of escalation requests do not end up identifying any deterioration in care, around two fifths (40%) *do*!

While on one level this is a low proportion of all NHS treatments, for the patients & families involved this looks life saving!
